For Patients
You must be referred to our office by your family physician, walk-in clinic physician or another specialist. You must present a valid Ontario Health Card or your up-to-date Interim Federal Health paperwork at the time of your appointment. Should your health card or IFH paperwork be expired, you will need to update the information and rebook your office appointment. We do not see uninsured, out of country or private patients in our practice.
If you do not have a family physician, you can reach out to Health Care Connect, a program launched by the Ministry of Health and Long Term Care to help residents of Ontario without a family health care provider, find one in your community. To be eligible for this program you cannot already be enrolled with a family doctor, and you must have a valid Ontario Health Number. For any questions regarding this program please call 1-800-445-1822 or register online at www.ontario.ca
If you have seen one of our surgeons in the past, but now wish to be seen for a new or different problem, you must see your family doctor first. Additionally, if it has been more than one year since your last appointment, you must be re-referred as required by OHIP rules.
Once a referral has been received in our office, it will be triaged by the surgeon prior to an appointment being booked for you. Once triaged, a staff member will then contact you directly to book an appointment in our office or at the hospital (as triaged by the surgeon). You will be scheduled for the first available appointment with the surgeon.
At this time, we do not see patients regarding the following:
- pediatric patients (no one under the age of 18 years)
- skin lesions
- hidradenitis
- head and neck (including thyroid problems)
- circumcision
- ingrown toenails
- follow-up for surgical issues done by another surgeon/Centre
- weight loss surgery/Lap Band
Referrals sent to us regarding any of the above will be returned to your family doctor to be re-directed to the appropriate physician.
